refactor: polish and optimise before 1.0.0

Clean-up and optimisation pass before 1.0.0, one change per commit, each with a test where behaviour changes. The content report drops rows for bundles no longer checked and refreshes when a draft is deleted. drush vale:install makes Vale executable before it replaces the old binary. Failed Vale runs and probes are not cached or repeated in a request. The check endpoint's slots cover its probes. Under publish-only, a paragraph's error is listed once. A field's paragraphs load in one query. The README, tests and Drupal 10 hook wrappers are tidied to match.
