Issue #3583789: Fix Entity MalformedException for cloned/new entities

  • Handle new entities gracefully by setting source to NULL
    • Wrap toUrl() call in try/catch for EntityMalformedException
    • Wrap token replacement in try/catch to handle tokens that call toUrl()
    • For failed token replacement, fall back to pattern without menu-link tokens
    • This allows Quick Node Clone support while still validating for duplicates
