Fix TFA full set up not working

Applying patch by marcellinostroosnijder to the issue fork

Tested the changes and confirm that these changes work.

  1. Enable 2 plugins: recovery codes and TOTP.
  2. Setup TOTP, you will no longer see a second TOTP seed and new QR code, and will be redirected back to the next validation plugin.
