bug: #3028070 Honor explicit width and height for both providers

Width was always discarded and recalculated from height whenever height was present, even when width was also given explicitly, making it impossible to embed a real 16:9 video at its actual dimensions. The same defect existed in Vimeo's resolver too, not just YouTube's as the issue title suggests, confirmed against the original Drupal 7 source.
