Issue #3360629: Add a checkbox to use a session cookie

vastr requested to merge issue/modal_page-3360629:3360629-ShowOnlyOnce_session_cookie into 5.1.x

Add a checkbox in extra part to use a show only once based on a session cookie. In case you don't want to use the checkbox "Don't show anymore", or don't want to use the "Show only once" option where cookie is set for 365*20 days, and don't want to display the modal on every page.

Show only once and session cookie can not be used at the same time.
