Issue #3519314: Add title and description fields to the settings

thanks @robloach! This definitely improves the UX. Could you also add the new title and description keys to config schema in llmstxt.schema.yml?

... and introduce an update hook that initializes these values either with an empty string or with other sane defaults to eliminate config schema validation errors.
