Issue #3388913 Refactor MediaLibrarySelectForm placeholders to use entity Ids

Placeholders built from row indices result in non-unique placeholders. This causes caching to break paged records of media items in the MediaLibrarySelectForm. This fix replaces row indices with entity Ids.
