Issue #3533072: Removed form_base_test.inc and moved test_form_id into TestForm::buildForm()

Moved test_form_id function to FormBuilderTest::testFormId() and removed the form_base_test.inc

Updated the function calls in FormBuilderTest class. Closes #3533072
