3613098: import failures are passed along to user, not just logged

3613098: updated CrossrefDoiEncoder and CrossrefReferenceNormalizer so on fetch failure a marker entry is appended to the decoded results, the failure travels into the import batch instead of being discarded, and the failures are reported along with the successes.
